I grant Modern Academy of Music permission to use photos and videos of me / my child for promotional purposes, including but not limited to brochures, calendars, flyers, videos, websites, and social media, provided that no identifying name or other personal information of the student is disclosed.

I understand and acknowledge that I am responsible for arriving on time for my stage rehearsal. I further understand that the auditorium charges an additional $250 per hour for extended rental time, and that if the show runs late due to my late arrival, I will be financially responsible for any additional auditorium rental fees incurred.

I understand and acknowledge that I must return this form no later than April 5, 2026, which is the final registration deadline.
