2023 Participation form

I will be participating in MAOM WINTER SHOW on DECEMBER 10, 2023 (rehearsal on the stage at 1:30 p.m., show at 4:00 p.m.)

Please initial to indicate your understanding of, and agreement with, the following (students not participating need only initial the first item):

Students must make every reasonable effort to attend all lessons and rehearsals before the recital.

The $50 Participation Fee are due with this form.

I will be participating in MAOM Kids show group performance (Ages 5-17) and will attend 3 group classes-rehearsals for group performances on November 25, December 2, December 9, 2023

For ages 5-7 y.o. at 11:30 a.m.-12:30 p.m., for ages 7-12 y.o. at 12:30 a.m. – 2:00 p.m., For ages 12-17 y.o. at 10:00 – 11:30 a.m. (rehearsals are required to attend to perform on the recital with a group songs)

All fees are non-refundable.

All costumes for group songs will be provided by MAoM to use on the stage for free and should be returned to your named case after your group performances on December 10, 2023. If costumes will not be returned on December 10. 2023 you will be charged $50 for each costume

If your child participates in group performances she/he must bring to the show his/her own: Logo T-shirts, white plain sneakers, white plain high socks for girls and white plain pants for boys
