Youth group (Ages 5–10 y.o.): 11:45 PM (Alexandra, Jolina, Briela, Gregory, Karsyn, Elizabeth, Anya W., Anna B., Masha B.)
Youngest group (Ages 5-6 y.o.): 1:45 PM (Eliza, Daniel)

Students must make every reasonable effort to attend all lessons and rehearsals before the show.

The $80 Participation Fee are due with this form.

I will be participating in MAOM Kids show group performance (Ages 5-17) and will attend 4 group rehearsals for the WINTER SHOW 2025 by MAoM.

The $60 Kids show group participation fee are due with this form.

All fees are non-refundable.

All costumes for group songs will be provided by MAoM to use on the stage for free and MUST be returned to your named case after your group performances on December 21, 2025. If costumes will not be returned on December 21, 2025 you will be charged $50 for each costume.

If your child participates in group performances she/he must bring to the show his/her own: Logo T-shirts, white plain sneakers, white plain high socks for girls and white plain pants for boys.

Please return this form no later than November 15, 2025 (final registration deadline).

Please arrive on time for your stage rehearsal to avoid late arrival fees.
The auditorium charges an additional $250 per hour for extended rental time.
If the show runs late due to a performer’s late arrival, the financial responsibility for the extra auditorium rental will be on the late-arriving performer.
